CNF Needs Assessment Survey

Dear CNS Members,

Each year, Child Neurology Foundation (CNF) conducts an annual needs assessment survey to better understand the needs of parents and caregivers caring for children living with a neurologic condition. These results in turn help drive CNF’s programming efforts.

This year, for the first time ever, we are also looking for your feedback, because we truly believe it is imperative in considering future programming that we have a better understanding of the physician experience. Your feedback will help us develop programming to support you and your patients.

The results of each survey are imperative in helping set future direction for CNF as we look to celebrate our 20th anniversary in 2021 and set direction for our next 20 years.

Share Our Parent/Caregiver Needs Assessment – Open through Friday, April 23rd
And we kindly ask for your help in sharing our annual parent/caregiver needs assessment survey with your patients too, using the following suggested copy:
Child Neurology Foundation is looking to hear from caregivers of kids living with neurologic conditions about their experiences. Your feedback will be used to directly influence the resources provided to the community. The survey is open until April 23rd and all eligible respondents who complete it will be entered to win a gift card.

SHARE THE PARENT/CAREGIVER SURVEY

Anonymized results, key takeaways and proposed next steps from each survey will be shared this summer. And in both cases, your submission and each person you refer that completes a survey and marks you down as the referrer will get you entered into a drawing for a gift card as a small thank you for your time.
